Enclomiphene Results: What the Trials Measured

What enclomiphene trials measured for testosterone, LH, FSH and sperm counts at 2 weeks, 6 weeks, 16 weeks and 6 months, with every study cited.

Short answer: In published trials of oral enclomiphene citrate, enclomiphene “before and after” means lab numbers, not photos. A phase II trial reported that total testosterone rose within two weeks and averaged 604 ng/dL at six weeks on the 25 mg dose, and two phase III trials reported higher testosterone, LH and FSH at 16 weeks, with sperm concentration kept in the normal range (Wiehle 2013; Kim 2016).

This page reports what published trials measured, at the timepoints they measured it. It is not a prediction for any one person and not medical advice. Your provider sets the dose.

What does “before and after” mean for enclomiphene?

Enclomiphene trials don’t measure appearance. They measure blood and semen: total testosterone, luteinizing hormone (LH), follicle-stimulating hormone (FSH) and sperm concentration. So this page has no photos and no testimonials. It sets out the published numbers by timepoint.

All of these trials enrolled men with secondary hypogonadism, meaning low testosterone with low or normal LH. In the phase III trials, men had morning testosterone of 300 ng/dL or less and LH under 9.4 IU/L on two separate occasions (Kim 2016). For context on these numbers, see our page on normal testosterone levels.

What did the trials measure, and when?

TimepointStudyWhat the authors reported
Within 2 weeksPhase II, 48 men, enclomiphene 6.25, 12.5 or 25 mg vs testosterone gel (Wiehle 2013)Enclomiphene and testosterone gel both increased total testosterone within two weeks
6 weeks (day 42)Same phase II trialMorning total testosterone 604 ± 160 ng/dL on 25 mg vs 500 ± 278 ng/dL on gel, not significantly different (p = 0.23); LH and FSH rose above the normal range on enclomiphene, LH was suppressed on gel
3 monthsPhase IIB, 12 men switched from testosterone gel (Kaminetsky 2013)A significant rise in testosterone in both groups; sperm counts raised in 7 of 7 men on enclomiphene
16 weeksTwo phase III trials, overweight men aged 18 to 60 (Kim 2016)Total testosterone increased in all treatment groups; LH and FSH increased on enclomiphene and decreased on gel; sperm concentration stayed in the normal range on enclomiphene, with a marked drop on gel
6 monthsPhase IIB (Kaminetsky 2013)Total testosterone 525 ± 256 on enclomiphene vs 545 ± 268 on gel; sperm counts raised in 6 of 6 men on enclomiphene, at 75 to 334 million/mL
1 week to 1 month after stoppingWiehle 2013; Kaminetsky 2013LH and testosterone effects lasted at least one week after stopping; testosterone fell back to pretreatment values one month after treatment ended

Weeks 2 to 6: what did the phase II trial report?

Wiehle and colleagues compared three daily doses of enclomiphene citrate (6.25, 12.5 and 25 mg) with AndroGel testosterone gel in 48 men. They measured testosterone and LH every hour for 24 hours on day 1 and again on day 42. The authors reported that all three doses increased testosterone, that enclomiphene increased LH and FSH while gel suppressed LH, and that enclomiphene did not significantly affect TSH, cortisol, lipids or bone markers. Both treatments lowered IGF-1, more so on enclomiphene (Wiehle 2013).

3 to 6 months: what did the small phase IIB trial report?

Kaminetsky and colleagues studied 12 men who had been using testosterone gel. After stopping gel, their morning testosterone averaged 165 ± 66. The authors reported a significant rise by 3 months on both enclomiphene and gel, sustained to 6 months. Only the enclomiphene group showed increased LH and FSH. The gel did not raise sperm counts above 20 million/mL for any of the five men on it at 3 months (Kaminetsky 2013). With 12 men, this is a very small study.

16 weeks: what did the phase III trials report?

Kim, McCullough and Kaminetsky reported two parallel randomized, double-blind, placebo-controlled phase III trials (ZA-304 and ZA-305). They compared two doses of enclomiphene citrate with AndroGel 1.62% over 16 weeks. The authors concluded that enclomiphene increased total testosterone, LH and FSH and kept sperm concentration in the normal range. They reported that gel also raised testosterone, but sperm counts were not maintained (Kim 2016).

What happened to sperm counts in the trials?

In addition to the phase III results above, a separate phase II trial (ZA-203) reported that sperm counts were conserved on enclomiphene while it raised morning testosterone and LH to levels similar to 1% testosterone gel (Wiehle 2014).

A 2025 meta-analysis that pooled clomiphene and enclomiphene trials reported that, compared with testosterone gel, the risk of a sperm concentration below 15 million/mL was lower with these drugs (risk ratio 0.10; 95% CI 0.04 to 0.23) (Hohl 2025). Our page on TRT and fertility covers testosterone therapy and sperm production in more detail.

What does the 2025 meta-analysis report on testosterone?

Hohl and colleagues pooled 10 randomized trials of clomiphene or enclomiphene in men with testosterone of 300 ng/dL or less, with follow-up from 2 to 30 weeks. Compared with placebo, they reported average increases of 273.76 ng/dL in total testosterone, 4.66 IU/L in LH and 4.59 IU/L in FSH. They found no significant difference in total testosterone between these drugs and testosterone gel (Hohl 2025). Because the analysis groups clomiphene and enclomiphene together, its numbers are not specific to enclomiphene. For how the two differ, see enclomiphene vs clomiphene.

How long does enclomiphene take to work?

The trials answer this only in terms of lab values. The earliest reported change is within two weeks, when the phase II trial reported an increase in testosterone. The same trial reported results at six weeks, and the phase III trials reported theirs at 16 weeks (Wiehle 2013; Kim 2016). The abstracts cited here don’t report when, or whether, symptoms changed. A follow-up hormone panel shows a provider your own numbers.

What hasn’t been studied?

The longest treatment period in the studies on this page is six months (Kaminetsky 2013), and the meta-analysis lists follow-up of 30 weeks at most (Hohl 2025). None of these sources reports long-term outcomes. To see how enclomiphene compares with testosterone replacement on what is known, read enclomiphene vs TRT.

How does enclomiphene work with PrescribedRX?

It starts with a hormone panel. The $69 hormone panel covers total testosterone, LH, hematocrit and PSA, drawn at a Quest lab near you, and a licensed provider reads the results. Enclomiphene through PrescribedRX is compounded. Compounded medications are prepared by licensed pharmacies and are not FDA-approved. A licensed provider decides whether it is appropriate for you, and the plan is charged only if it is prescribed. Your provider sets the dose. Each renewal comes with a $69 follow-up panel, billed separately.

  • Enclomiphene (compounded): $387 ($129/mo), billed every 3 months, charged only if prescribed
  • Hormone panel at Quest: $69 per panel

Start with the $69 hormone panel, or see the enclomiphene plan for details. You can also compare TRT costs or browse all testosterone treatments. Self-pay, no insurance, free priority shipping, and availability varies by state; the eligibility check confirms yours.
