There is no Levels affiliate program. Not a closed one, not an invite-only one. Levels publishes no affiliate program, no commission structure and no revenue share of any kind.

What actually exists
Two things, and neither pays a commission.
A partnership request form
Levels’ support site carries a page last updated 6 March 2026 which reads: “If you’re a content creator, health and wellness practitioner, or otherwise interested in partnering with Levels, submit a request using the form below.”
That is an inbound enquiry funnel. No program structure, no rate, no terms, no platform. Whether anything sits behind it is not public.
A referral discount, where the referrer earns nothing
A support article last updated 15 July 2026 states: “When you refer someone to Levels, they get $25 off their membership.”
Read that carefully. It describes only what the referred person receives. There is no cash, credit or commission to the person doing the referring published anywhere on Levels’ own domain or support site. The article further specifies the $25 is “a one-time use promo code, not a gift card or stored credit” and that “any unused portion does not carry over as a remaining balance.”
The referrals category on their support site contains exactly two articles, the referral one and the partnership request one. There is no third, affiliate-specific article.
Two traps to avoid
Name collision. Several unrelated companies use “Levels.” Any affiliate commission you find attributed to “Levels” is more likely one of those than the continuous glucose monitoring company. Check the domain before you trust a figure.
The domain moved. Levels has largely migrated from levelshealth.com to levels.com, and the support site is now on a subdomain of the newer one. Old pages and old directory entries may point at the previous domain.
No third party directory listing claiming a Levels affiliate rate was found in this research. Searches surfaced only competitor programs in the same category. That absence is itself informative: if directories had a number, they would publish it.
What to do instead
If your audience is interested in metabolic tracking and lab-driven health, the adjacent programs that do publish terms are worth comparing. Function Health publishes a $25 member referral fee and runs on Impact, though its creator rate is also unpublished. If you specifically want the Levels relationship, the partnership form is the only route, and you would be negotiating rather than joining a published program. Ask for the rate, the attribution window and the payment schedule in writing before producing anything.
