Compounded Semaglutide Online: Cost, Process and Safety
PrescribedRX is a telehealth platform; prescriptions are written by licensed clinicians and filled by licensed partner pharmacies.
You can get compounded semaglutide online through PrescribedRX by completing an online intake that a licensed provider reviews. If the provider prescribes it, a licensed partner pharmacy fills it and ships it free by priority mail, at $420 for a 3-month plan (about $140 a month). Compounded semaglutide is not FDA approved, so read the safety and legal section below before you decide.
Is compounded semaglutide FDA approved?
No. The FDA says it does not review compounded drugs for safety, effectiveness or quality before they are sold. Only the brand-name products made by Novo Nordisk, such as Wegovy and Ozempic, have gone through FDA approval.
The rules have also changed. Semaglutide was on the FDA drug shortage list from 2022, which let pharmacies make copies on a wide scale. On February 21, 2025, the FDA determined the semaglutide shortage was resolved. The FDA then gave state-licensed (503A) pharmacies until April 22, 2025, and outsourcing facilities (503B) until May 22, 2025, to wind down shortage-based compounding, as explained in its policy update for compounders.
Compounding has not been banned outright, but it is now limited. Under federal law, a 503A pharmacy can compound for an individual patient with a prescription, and it may not regularly make products that are “essentially copies” of an approved drug. One exception is when a prescriber determines and documents that a compounded product contains a change that makes a significant difference for that patient. The legal picture is nuanced and can differ by state, so we suggest you ask your provider how your prescription meets these rules.
The FDA has also reported adverse events linked to compounded semaglutide, some needing hospital care and some tied to dosing errors. It has warned about products made with semaglutide salt forms (such as semaglutide sodium or acetate), which it calls different active ingredients from the approved drugs. In 2025 and 2026 the FDA sent warning letters to telehealth companies over how they marketed compounded GLP-1s, as noted on its GLP-1 concerns page (last updated October 1, 2026).
What is compounded semaglutide?
Semaglutide is a GLP-1 receptor agonist. It acts like a gut hormone that helps control appetite and blood sugar, which is why it is used for type 2 diabetes and chronic weight management. MedlinePlus, from the National Institutes of Health, lists its approved uses.
Compounded semaglutide is prepared by a licensed pharmacy for a specific patient’s prescription instead of being mass-produced by the brand manufacturer. Because it is not FDA approved, its makeup, strength and quality are not checked by the FDA the way brand-name products are. It is not a generic, and it should not be treated as interchangeable with Wegovy or Ozempic.
Who is semaglutide for?
Your provider decides whether any weight-loss medicine fits you. For context, the FDA-approved brand Wegovy is labeled for adults with a body mass index (BMI) of 30 or higher, or 27 or higher with at least one weight-related condition such as high blood pressure, type 2 diabetes or high cholesterol, used along with a reduced-calorie diet and more physical activity (Wegovy prescribing information). That label applies to Wegovy. It is a common reference point, not proof that a compounded product works the same way.
Semaglutide may not be right for you if you:
- Have a personal or family history of medullary thyroid cancer or Multiple Endocrine Neoplasia syndrome type 2 (MEN 2).
- Have had pancreatitis, or have serious kidney, gallbladder or stomach problems.
- Are pregnant, breastfeeding or planning a pregnancy.
- Already take another GLP-1 medicine or insulin. Tell your provider about every medicine you use.
How does getting semaglutide online work?
- Take the quiz. The eligibility quiz checks whether the program is offered in your state. Availability varies by state.
- Complete your intake. You answer questions about your health history, weight, current medicines and goals.
- A licensed provider reviews it. A state-licensed clinician working through Telegra MD reviews your answers, typically within 48 hours, and may ask follow-up questions. The provider decides whether to prescribe and sets the dose.
- A partner pharmacy fills it. If prescribed, a licensed partner pharmacy prepares your medication and ships it with free priority shipping. PrescribedRX is not a pharmacy.
- Ongoing care. Your plan renews every 3 months. You can cancel anytime. Tell your provider about any side effects or questions.
How much does semaglutide cost online?
PrescribedRX is self-pay only and does not bill insurance. The price is the same at every dose. Here is how it compares with brand-name self-pay and list prices as of October 2026.
| Option | FDA approved? | Price | About per month |
|---|---|---|---|
| PrescribedRX compounded semaglutide | No | $420 for 3 months (2 months + 1 free) | $140 |
| PrescribedRX compounded tirzepatide | No | $597 for 3 months | $199 |
| Wegovy injection, NovoCare Pharmacy self-pay | Yes (weight management) | $199/mo for the first 2 months (new patients, through Dec 31, 2026), then $349/mo | $349 |
| Wegovy pill, NovoCare Pharmacy self-pay | Yes (weight management) | $149 to $299/mo depending on dose | $149 to $299 |

Brand self-pay prices are as listed on Novo Nordisk’s Wegovy and Ozempic NovoCare pages in October 2026. List prices are as reported by Fierce Pharma. Novo Nordisk has announced that list prices for most doses will drop to $675 a month on January 1, 2027. Your insurance may cover a brand-name drug, which can change the math.
One more thing to know: the “per month” figure is the plan price divided by 3. See our pricing page for every plan.
What are the common side effects of semaglutide?
According to MedlinePlus and the Wegovy label, the most common side effects are stomach-related:
- Nausea, vomiting and diarrhea
- Constipation, stomach pain and heartburn
- Headache and tiredness
Serious but less common risks include pancreatitis, gallbladder problems, kidney problems, severe allergic reactions, vision changes and low blood sugar in people who also take diabetes medicines. The brand-name product carries a boxed warning about thyroid C-cell tumors seen in animal studies. Get medical help right away for severe stomach pain that spreads to your back, a lump or swelling in your neck, trouble swallowing or trouble breathing. Your provider sets the dose and how it changes over time.
Semaglutide vs tirzepatide: which should you ask about?
Semaglutide acts on one hormone pathway (GLP-1). Tirzepatide acts on two (GLP-1 and GIP). At PrescribedRX, compounded semaglutide is the lower-cost starting point at about $140 a month, and compounded tirzepatide is about $199 a month. Neither compounded version is FDA approved. Your provider can help you weigh the choice based on your health history and how you tolerate treatment.

Compounded semaglutide FAQ
Is compounded semaglutide the same as Wegovy or Ozempic?
No. Wegovy and Ozempic are FDA-approved brand products. Compounded semaglutide is not FDA approved, and the FDA has not reviewed it for safety, effectiveness or quality. Do not assume results or side effects will match the brands.
Is compounded semaglutide still legal after the shortage ended?
It can be, in limited cases. After the shortage ended, the FDA’s grace periods closed in April and May 2025. Pharmacies can still compound for individual patients under specific federal and state rules, such as when a prescriber documents a meaningful clinical difference for that patient. The FDA is actively enforcing these rules, so the situation may keep changing.
Do I need insurance?
No. PrescribedRX is self-pay only. We do not bill insurance, and you pay the same price at every dose.
How often am I billed?
The semaglutide plan is $420 for a 3-month supply (2 months plus 1 free). It renews every 3 months. You can cancel anytime.
Is it available in my state?
Availability varies by state. The quiz will tell you before you complete the full intake.
Will I lose weight?
Results vary, and no one can promise a specific outcome. Semaglutide is meant to be used with a reduced-calorie diet and more physical activity. Your provider will check in on your progress and side effects.
