The short answer
You can legally buy testosterone online in the US. You cannot legally buy it without a prescription.
Testosterone is a Schedule III controlled substance. That means a DEA-registered prescriber, a licensed pharmacy, and real clinical justification. Any site offering it without those things is selling you something illegal, and often something that is not testosterone.
What Schedule III actually means for you
Congress placed anabolic steroids in Schedule III by statute. Testosterone sits there alongside them.
Three practical consequences:
A prescription is required, issued by a practitioner registered with the DEA, and dispensed by a licensed pharmacy.
Refills are capped. Federal rule 21 CFR 1306.22: no Schedule III prescription may be refilled more than five times, or more than six months after it was written. This is why a legitimate TRT provider makes you re-test and re-consult periodically. It is not upselling. It is the law.
Possession without a valid prescription is a federal offence under 21 U.S.C. 844 — up to a year for a first offence. Distribution carries up to ten years.
That last one is not theoretical. In August 2026 the DEA charged four Arizona residents with trafficking Schedule III anabolic steroids, including products containing testosterone propionate, isocaproate, decanoate and phenylpropionate, shipped across state lines. The maximum penalty cited was ten years.
Can a doctor prescribe testosterone over telehealth?
Yes, currently — and this is one to check the date on.
The Ryan Haight Act normally requires an in-person examination before a controlled substance can be prescribed. Since 2020 that requirement has been waived, and the waiver has been extended four times.
As of September 2026: a DEA and HHS rule published on 31 December 2025 permits DEA-registered practitioners to prescribe Schedule II to V controlled substances via telemedicine without a prior in-person exam. It is effective through 31 December 2026.
The permanent replacement — a special registration framework proposed in January 2025 — is at the White House for final review, with action expected around November 2026. Its text has not been published.
What that means for you: telehealth TRT is legal today. The rules governing it may change at the end of this year. A provider who has been doing this properly will be registered and licensed either way.
What a legitimate online provider does
Licensure in your state. The prescriber must be licensed where you are, not just where they are. Ask, or check.
DEA registration. Required to prescribe any controlled substance, including via telemedicine.
Real lab work before prescribing. The AUA standard is two early-morning total testosterone measurements on separate occasions, below 300 ng/dL, alongside symptoms. A provider who prescribes from a single random-time reading — or from no reading at all — is departing from both major US guidelines.
Ongoing monitoring. Testosterone every 6 to 12 months. Haematocrit, with action at 54%. PSA, with urology referral if it rises more than 1.4 ng/mL in the first year or passes 4.0 ng/mL.
A pharmacy you can verify. NABP Digital Pharmacy Accreditation is the recognised US mark and NABP publishes the list.
LegitScript certification. This is the credential that lets a healthcare company advertise on Google, Meta, Microsoft and others. It requires appropriate licensing, valid prescriptions, and certified partner pharmacies. Its absence on a site running paid ads is worth noticing.
How the industry actually performs
Worth knowing before you choose anyone, including us.
A 2022 study assessed seven direct-to-consumer testosterone platforms. One of seven measured morning testosterone twice as guidelines require. Six of seven offered testosterone to a simulated patient who had normal levels and wanted to preserve fertility.
A 2026 review of 200 men newly prescribed testosterone found 12% had a complete diagnostic workup.
Legal and thorough are not the same thing. Most of the market clears the first bar and not the second.
The risk of buying without a prescription
Beyond the legal exposure, there is what is in the vial.
Brazilian federal police seized 345 black-market products and analysed them by mass spectrometry. About 42% were counterfeit. Broken down by format, the figure for oil solutions — which is what injectable testosterone is — was 65.2%. A further 11% contained the wrong dose.
That is a Brazilian seizure cohort, not a US one, and we are not going to pretend otherwise. But injectable testosterone from an unregulated seller is an oil solution of unknown contents, produced by someone with no obligation to tell you the truth, injected into your muscle.
There is also no monitoring. The side effects that matter most on TRT — rising haematocrit, rising PSA, rising blood pressure — are silent until they are not. They are caught by blood tests nobody is ordering.
Red flags
The DEA warning list for illegal online pharmacies:
- Sells prescription drugs without requiring a prescription
- Prices far below market
- Prices in a foreign currency
- No verifiable state pharmacy licence or DEA registration
- Damaged packaging or foreign-language labelling
- Missing or expired expiry dates
- Medication that looks different from what a pharmacy supplies
TRT-specific additions:
- No bloodwork required before prescribing
- No named, licensed prescriber
- No statement of which states they can treat
- No prescription needed claims
- Ships from overseas
- Payment by cryptocurrency or wire transfer only
- Vials labelled research use only or not for human consumption
- Any claim that testosterone is not a controlled substance

- A US-licensed provider reviews it and orders the lab work needed.
- If your levels and symptoms support treatment, you are prescribed a plan at $387 every 10 weeks — the same price at every dose.
- If they do not, you are not prescribed. The assessment fee is refundable if you do not qualify.
- Your prescription is dispensed by a licensed pharmacy and shipped in discreet packaging.
- You re-test and re-consult periodically, because Schedule III prescriptions expire.
We cannot treat patients in every state. If you are somewhere we are not licensed, we will tell you at the assessment rather than after you have paid for a plan.
Common questions
Is it legal to buy testosterone online? Yes, with a valid prescription from a licensed, DEA-registered provider, dispensed by a licensed pharmacy. Without one, no.
Can I get testosterone prescribed without seeing a doctor in person? Currently yes. Telehealth prescribing of controlled substances is permitted through 31 December 2026 under a federal rule. The permanent framework is still being finalised.
What happens if I buy testosterone without a prescription? It is a federal offence, and roughly two thirds of black-market injectable products analysed in one seizure study were counterfeit or wrongly dosed.
Why do I have to keep getting blood tests? Two reasons. Schedule III prescriptions cannot be refilled more than five times or beyond six months. And the main risks of TRT are detected by blood work before you feel them.
How do I check a provider is legitimate? Confirm the prescriber is licensed in your state, the pharmacy is NABP-accredited, and lab work is required before any prescription is issued.
